A 23-year-old male was admitted to our hospital with complaints of shortness breath, grade-III, chest pain, and palpitation for the last 3 months. He had one episode hemoptysis months back. a known case primary antiphospholipid antibody syndrome (APS) bilateral chronic pulmonary thromboembolism thromboembolic hypertension (CTEPH) presence anticardiolipin antibody. The patient no history joint hematuria, photosensitivity. on domiciliary oxygen therapy at 1 L/min maintain an saturation 93%, riociguat, ecosprin, hydroxychloroquine, enoxaparin. referred thromboendarterectomy (PTE).
